AFFORDABLE COPAYS, IMPROVED VISION

We offer a Vision plan with low copays for eye exams and additional eye-health services. The plan provides an annual allowance of $150 for frames or contact lenses and $150 for vision services. In addition, Planstin waives all copays when services are obtained at wholesale clubs like Sam's Club, Costco, and Walmart. *To enroll in the Vision plan, members must be enrolled in a Planstin Base Health plan. No other participation requirements apply.
